Nurse-led Care Services | Case Studies

John has lived with Multiple Sclerosis for more than 20 years, however, over time, his condition has gradually worsened leading to complete immobility and highly complex health needs.

Having had a rather significant fall in 2024 and subsequent deterioration, John had spent a prolonged period of time in a care home.

However, the care home environment simply didn’t provide John with the support or level of clinical responsiveness that he required. That, paired with John’s autonomy, emotional wellbeing and value of family life, he expressed a clear desire to return home – however this needed to be done safely and with clinical oversight.

Interventions Supporting John’s Health, Comfort and Daily Living

Keen to return home, John and his family worked closely with the clinical team at Radis to make this happen.

The Radis team completed a comprehensive assessment including respiratory risk and infection history, pressure damage, wound management, pain control and medication complexity.

Following on from this, a bespoke nurse-led care plan was developed and implemented in collaboration with John’s family and wider medical professionals. Due to his condition, John also receives professional bowel care, further supporting his health and medical needs.

Because of this bespoke plan, John was able to return home and is now supported on a 24/7 basis by the Radis care team.

How Nurse-Led Care Improves Outcomes for John

Under nurse-led oversight, early signs of respiratory infection are identified and escalated promptly, use of cough assist, nebulisers, inhalers, and chest physiotherapy is clinically guided and hospital admissions have reduced through early intervention.

As John is at high risk of pressure injuries, clinical leadership ensures repositioning regimes are appropriate and consistently followed, pressure area checks are documented and reviewed, wound changes are escalated immediately and equipment such as profiling beds and dynamic mattresses are monitored for effectiveness.

Since receiving Nurse-led Care from Radis, John’s clinical stability has improved, anxiety has reduced, hospital admissions have decreased and John feels heard, respected, and safe.

Returning home has allowed John to enjoy family time with his daughter, and he now talks about the future again.

The clinical team’s support has resulted in the following:

  • Improvement of clinical stability
  • Reduction in anxiety
  • Decreased hospital admissions
  • John feels heard, safe and respected

Nurse-led care encourages and supports a truly person-centred approach and system. By combining clinical leadership, a deep personal understanding and skilled care delivery, Radis has enabled John to live at home safely, dignity, autonomy and hope – something that may not have been achievable in a different setting.

Remaining at home allows people to maintain a level of independence and stability, prioritising emotional wellbeing as well as physical and clinical needs.

John said, “I feel safe at home because the nurses know what they’re doing – and the carers do things the way I like. That’s what makes it work.”
