Nurse-led Care Services | Case Studies

Cameron lives with quadriplegic cerebral palsy, which significantly impacts all four of his limbs as well as his overall mobility, muscle control, and ability to carry out daily activities independently. In addition to cerebral palsy, Cameron also has epilepsy, which requires careful medication management and monitoring.

Cameron’s monitoring and care is carried out by highly qualified staff, with the clinical team at Radis Community Care training Support Workers to a high standard.

Thanks to the Nurse-led Care Service by Radis, Cameron is able to access the community, spend time socialising and pursuing his hobbies whilst receiving the long-term care he requires.

Intervention Supporting Cameron’s Health, Comfort and Daily Living

Due to swallowing difficulties and nutritional requirements, Cameron has a PEG tube, through which all medications, hydration, and nutritional support are administered. This requires nurse-led oversight and clinical procedures, including daily PEG site care, medication administration, and monitoring for any signs of infection or complications.

The nurse-led framework ensures that Cameron’s clinical needs are managed and overseen safely whilst maintaining consistency in his daily care and requirements.

As a wheelchair user, Cameron requires hoist transfers, including for moving between his bed, wheelchair, and shower chair. He uses specialist equipment including a moulded wheelchair, profiling bed, and sleep system to maintain correct positioning and reduce the risk of pressure damage.

Clinical Importance of Nurse-Led Care

Under the nurse-led care approach, the clinical team are able to monitor Cameron closely without being invasive or taking away his independence.

Cameron’s care is built around:

  • Early recognition of health changes or deterioration, reducing the risk of hospital admissions and intervention
  • Clinical oversight
  • Clear escalation pathways
  • Multidisciplinary team coordination, ensuring the right professional input at the right time

With nurses leading Cameron’s care, any changes are identified and acted upon promptly, managed safely whilst prioritising socialisation and independence without compromising on care quality.

Although Cameron has significant physical care needs, he is actively involved in decisions about his care and daily routine, ensuring he feels happy and confident.

Cameron’s support highlights the importance of a person-centred, nurse-led approach, where complex clinical needs are managed safely while focusing on independence, communication development, education, and quality of life.

Life Now

Cameron attends college, where he participates in education designed to support individuals with moderate to severe learning difficulties. Supporting Cameron’s educational development is an important part of his bespoke care package and supports feelings of independence, requiring coordination between the clinical team at Radis, therapists, and the college team.

Cameron enjoys an active social life when supported. He particularly enjoys watching television, going to the cinema, participating in sports activities, and spending time with family and friends.

With the support of his care team, Cameron is also able to further his independence by being able to access the community by using a mobility vehicle, ensuring he remains socially connected and engaged in activities he enjoys.

Through consistent support from his care team, Cameron is empowered to pursue his goals, maintain meaningful relationships, and participate fully in the activities that matter most to him.
