Nurse-led Care Services | Case Studies

Barry is a 79-year-old retired army officer, and after a fall in March 2024, he was left with a C6/7 spinal fracture resulting in incomplete tetraplegia, respiratory weakness and significant functional loss. On top of this, Barry was also diagnosed with Autonomic Dysreflexia, also known as AD – a condition that can affect people with certain spinal injuries, that happens when the body overreacts to the likes of a full bladder or bowel.

Barry’s early care was somewhat fragmentated and involved a number of teams with limited coordination. However, in late 2024, Radis Community Care took over his care under a nurse-led framework, working with a number of professionals to create a comprehensive person-centred care plan.

Interventions Supporting Barry’s Health, Comfort and Daily Living

The clinical team worked with Barry to support a variety of his needs, including PEG feeding, bowel and catheter care, respiratory management and mobility assistance. Working closely with the care and operational staff, the clinical team prioritised stability, safety and continuity throughout his care plan.

Key interventions included the following:

  • Regularly basic observations and monitoring to proactively detect any signs of Autonomic Dysreflexia
  • Development of Standard Operating Procedures (SOPs) for PEG and bowel care, ensuring best practice and consistency
  • Regular monitoring to detect early warning signs
  • Ongoing skin integrity, hydration, and bowel reviews to prevent complications
  • Seamless coordination between nurses, physiotherapists, occupational therapists, speech and language therapists (SALT), and dietitians to deliver truly holistic care

Multiple teams at Radis worked together to ensure timely access to essential equipment, such as the power chair, airflow mattress and recliner. All of this was integral to enabling Barry and his wife Annette to plan their days with confidence and minimal disruption.

Rebuilding Confidence, Independence and a Sense of Normal Life

Radis Community Care has helped Barry to rediscover a sense of independence and control over his own life, being treated less like a patient and more like an individual.

Unsure as to how to navigate this new chapter in his life, Barry and the clinical team worked to celebrate every meaningful milestone – from self-directing personal routines to increasing time in his wheelchair.

Through consistent empathy, reassurance and structured goal setting, both Barry and his wife were guided every step of the way. The clinical team and the wider teams at Radis Community Care ensured Annette was included in every discussion and decision, supported with education, guidance and space to voice to her thoughts, ideas and concern.

Together, Barry and Annette described their relationship with the Radis teams as something much more personal than the standard care arrangement.

The couple said:

“We absolutely adore the Radis team. They’re not just here to care – they care about us. They understand our routines, our personalities and our space. We feel supported but never crowded.”

From a social and independence perspective, Radis Community Care and their integrated approach has enabled Barry and Annette to regain the balance between independence and security. This approach has now enabled the couple to enjoy the freedom to live their lives, providing them with the confidence to do so.

At home, Radis Community Care has restored both comfort and accessibility in conjunction with the occupational therapy and operational teams. This integrated approach enabled Barry’s PEG to be safely removed, allowing him to share family meals once again.

Barry and Annette’s family have been a constant source of encouragement and love throughout this entire journey. And today, Barry continues to set new goals; extending time in his wheelchair, setting up an adapted workspace, and even planning to watch a football match at the local club.

Clinical Success and the Future

Barry’s journey is a prime example of how the biopsychosocial model can be brought to life through effective, collaborative and compassionate teamwork from Radis.

The clinical team led Barry’s clinical coordination, training and governance whilst building trust and providing emotional support, which in turn improved clinical safety, confidence and communication.

Ensuring consistent daily care, Radis and the wider teams developed strong relationships with both Barry and Annette, enhancing comfort, reassurance and trust.

Radis ensured reliable scheduling, flexible support and the timely delivery of equipment, maintaining independence, privacy and normality at home.

Occupational therapists, physiotherapists, SALT and dieticians supported holistic recovery goals, PEG removal, mobility improvements and reintegration into daily life.

Since the implementation of nurse-led care, the following outcomes have been achieved for Barry:

  • No unplanned hospital admissions
  • Safe PEG removal with full return to oral intake in just 12 months
  • Stable respiratory function with no chest infections post-discharge
  • Improved medication adherence and positivity safety audit results
  • Enhanced quality of life and wellbeing (as reported by Barry and Annette)

Although Barry’s AD hasn’t been ‘fixed’, he has had zero episodes and is a situation that is being very well-managed despite its high-risk nature. Caused by a sudden surge in blood pressure, AD can be fatal, however our clinical team and Barry are confident that this is being managed as best it can be.

Barry’s story is a powerful reflection of what can be achieved when clinical expertise meets compassion.

“When we look back, it’s amazing to see how far Barry has come since leaving hospital. We couldn’t have done it without the incredible support from Radis Community Care as well as our family”, Annette reflects.
